Benefits available under this program include:
  • Guaranteed issue* for all eligible employees and dependents
  • Amounts of employee Life and AD&D coverage ranging from
  • $10,000 to $100,000 in $10,000 increments
  • Optional Life-Only benefits for Spouse and Children:
    • Spouse coverage available (contact for more information)
    • Children - $2,000 ($400 for children under 6 months of age). Coverage is available for children from 15 days to age 19; age 25 if child is full-time student.
Plans are comprehensive yet simple:
  • All employees must recieve the same amount of coverage (Age reductions may apply)
  • ADEA** life insurance amount reductions apply to your pre-determined amount of insurance based on age:
    • At age 65 your insurance coverage reduces to 65%
    • At age 70 your insurance coverage reduces to 40%
    • At age 75 your insurance coverage reduces to 25%
    • Dependent insurance coverage terminates at age 70
  • The plan must be sponsored by and 100% paid for by the employer
  • All employees under the program must work at least 20 hours per week.
  • All employees must have been employed for at least 2 months
  • Accidental Death & Dismemberment (AD&D) terminates at age 70
